More details on the 2 yellows no red incident in the Prem that I referred to:
21st August 2004 / Premiership / Southampton v Blackburn Rovers (3-2)
Referee Andy D’Urso (Billericay, Essex) issued Barry Ferguson (Blackburn) with a second yellow card during this game but failed to send him off. Ferguson had been previously cautioned for unsporting behaviour in the 82nd minute. When Southampton were awarded a penalty kick in the 91st minute, Ferguson punted the ball away. He received a second caution, for delaying the restart of the game but he was not sent off. The penalty kick scored; it was the winning goal.
The fact that two yellow cards had been issued to Ferguson was pointed out to the referee by the 4th Official, Keith Hill (Royston, Herts), before the kick off to restart but the referee apparently did not agree and so allowed Ferguson to remain on the field for the remaining two minutes of the game.
The FA Referees’ Committee suspended D’Urso for a period of 28 days, from 27th August to 23rd September, having found that he had ‘less than proficiently applied the Laws of the Game’. Ferguson received a one-match ban for his two yellow cards.
