They come out every month. As well as the importance of the game being factored into the calculation (friendlies count less than WC qualifers which count less than WC Finals etc), different zones get factored differently too.
As far as changes go:
- They have increased the ranking difference to reduce the impact of friendlies further than before.
- They have reduced the period of time over which games are included. Before it included matches over the preceeding 8 years, now it is 4 years.
- I believe they have remove consideration of home advantage.
